Job Description

RN are responsible for conducting (hands-off/in-field (reimbursement rate for mileage = .535/mile)) assessments for a state mandated tool. Assessments will take approximately 2.5-3 hours per assessment. Will complete approximately 2 cases per day (10/week).

Assessment is to evaluate ADL (activities of daily living) needs and determine if member meets requirements for nursing facility care. Assessments will be conducted in member’s homes, nursing facilities, and medical day care facilities.

Will travel primarily in certain counties but should be open and willing to travel throughout NJ, as needed. Assessments are conducted in the field but follow-up work can be done remotely from home.

RN’s will be given an iPad to complete assessments.

  • Passaic
  • Hudson
  • Bergen 
  • Essex 
  • Middlesex
  • Union 

Responsible for overall management of member’s case within the scope of licensure; provides supervision and direction to non-RN clinicians participating in the member’s case in accordance with applicable state law and contract; develops, monitors, evaluates, and revises the member’s care plan to meet the member’s needs, with the goal of optimizing member health care across the care continuum.

Qualifications

Minimum Education/Licensures/Qualifications:

  • Must have clear and active RN license in the state of NJ
  • At least 1 year of home health experience
  • 3-4 years of experience in working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management or similar role (or any combination of EDU/experience)
  • Comfortable with travel – must have reliable transportation (must be comfortable traveling outside of assigned county if necessary)
